Yeah, Denver held off for quite some time on weather, and so we just started getting winter as we entered in December, which is really late for us. So I can’t complain, even though I’m not a huge fan of the cold weather. My husband was sending me pictures from the airport this morning, and one thing DIA has, and I’m sure your Chicago airports have, is great snow removal equipment.
SPEAKER 05 :
Yes, we do. The de-ices are out and then sadly my shovels are out too. I came back and it snowed in actually for the holidays up in Ann Arbor, Michigan and it was just amazing. I came back to more than eight and a half inches and it was like, wow, whoever ordered this, they’re on my list and they’re getting cold for Christmas.
SPEAKER 07 :
Oh my goodness. All right, so what’s going on with you? Usually we talk about a book or some concept. What’s up today?
SPEAKER 05 :
Well, we are dealing with some interesting things with my students right now. I’m teaching a law and ethics class. I haven’t taught one of those in quite some time. And they are a sequence from a previous leadership course. And I’m a little concerned about some of the things that people really are throwing their leaders under the bus about. And I really want to make sure that it’s not just the leader that we’re talking about, because many of my students are struggling with the leader follower, right? You can’t be a leader if you don’t have at least one follower. And so we’re struggling with a little bit of understanding. It’s just not one directional. It’s that whole community of care you and I have talked about a lot of times, right? It’s not just up to one person, but… kindness and knowing someone has a voice in the conversation and being part of the collective and asking an opinion instead of telling an opinion. You know, things like that. The whole idea, because no one ever remembers what you say, but they always remember how you made them feel. And I’m just a little concerned my students are just struggling going, well, that’s a bad leader. I’m like, I don’t disagree, but what are you doing that could potentially contribute to this or contribute to its solution in identifying it? And many of them going, what do you mean? It’s up to me. It’s like, yeah, you’re part of this thing and we need to not sit so much on the sidelines. And so I’m trying to get that point across, Angie. So help me get the word out for
SPEAKER 07 :
Okay, so let’s talk about what you’re advising them to do because I do have two or three people in my life, one in particular that recently was told by some of us that you’re never wrong, you’re always the smartest guy in the room, you never admit when you’re wrong, and this person’s – let’s say feeling some fallout in their relationships because of this losing people some very important people even family because there is never an apology they’re never wrong the smartest guy in the room and I don’t understand how that benefits anyone I’ve never been that egotistical that I have to be the smartest guy gal in the room and so that I make others feel really small and really stupid and uninformed ill-informed just not as sharp as You know, I don’t want to make anyone feel like that. So I never understand when people like that boost their ego to be like, I’m right. You were wrong. I told you this was going to happen. And oh, I remember I called this and it’s like, how does that make you feel good? So what do you advise that your students do when they’re a leader who runs into people like that, that they have to manage or lead?
SPEAKER 05 :
or if they’re that person right and therein lies the challenges first we have to start at the end and back the truck up way up you have to look at the assumption of what kind of a leader does that person want to be and do they care the person that you’re referring to because everyone has this person in their family right everyone has that person on their team eventually that knows all be all and they’re just god’s gift to the human race in general Now, some of them want to be like that. For those who have that desire, you’re not going to convince them of everything. Stop trying. Because the point is, remember that nice adage, never argue with a fool because you might not know the difference? No, I haven’t heard that one.
SPEAKER 07 :
That’s actually hilarious.
SPEAKER 05 :
Yeah, because if you start arguing with someone and you already know the outcome, why engage them? Why even go? It’s like, you know what, if they are that way, then let them think that way and be gone because you have to decide on what your function in the conversation is, right? There’s two parts of the conversation, the sender and the receiver. If the receiver doesn’t care, then stop the conversation right then and there. If your purpose is to persuade them or to engage them, you’ve got to decide what’s the goal here. If you just want to chit-chat, then it doesn’t matter who’s right. If you want to persuade them or they are going to be important that they are always right, then why bother? So part of this is understanding the role before you ever engage. What’s the outcome? What’s the goal? You just want to chat and you just want to have a nice conversation. You just want to catch up. Then it really doesn’t matter to that extent. If you know that, oh, my gosh, this is the audience, then why bother? If you know the whole definition of insanity, do you keep doing the same thing over and over again? expecting different results. If you know what the result is going to be, why go there? You’re already going to know that the feelings are going to get hurt and they have to be number one and the rest of that. And if they want that and they like that, then you have the choice to walk away. You don’t have to play. And many people have that innate, but if they just understood what kind of a person that makes them and who does that. And, and we are the one that are outraged, but the fact is if they aren’t walk away, it’s not going to change other than it’s going to upset you and it’s not going to bother them. And so what’s the goal? So that’s the first reason is to, if you know the end result is why play? I had a friend call me out years ago and they just said, Cheryl, you’re the biggest hypocrite. You teach this. And yet here’s what happens in your family. And I’m like, and they were right. And so quite frankly, I took my bat and ball and I didn’t play in the sandbox anymore because I don’t believe in having miserable holidays. If you know that it’s going to be, and all this has been for the last 50 years, why are you going there? We already know the outcome. You just haven’t played your part. And if they don’t get the right script, It’s not enjoyable for anyone, like you said. So first of all, know your audience.
SPEAKER 07 :
And you and I have talked about this before. And at your family, it just became kind of butting heads when you would get together with your family each time you got together, right? Like disagreements.
SPEAKER 05 :
Right. Yeah, my family of origin, we already know you have the uncle who’s never wrong. You have the person who’s self-righteous. You have the and it was a all we’re going to do is argue. And there’s no safe subject because many subjects, you know, stay away from politics, stay away from this. But if only if the point is I have to be right, like you said, I have to make you feel small so that I can feel big. I don’t want to play. There is no winning that argument. So the question is, the only way to win that argument is to prevent it and not to have it. And so my point is with my family of origin, I don’t go. I don’t find sitting at family arguments to be the target and the wrath and the scapegoat and the. I want to go where I, which is where I was with other family members who celebrate me. We have a great time. There wasn’t, there was friendly ribbing. There was, you know, chaos. There were family jokes and things like that. That made it enjoyable. That made it fun. The arguing with the person who’s the know-it-all, fine. Leave the know-it-all, give them their thing, wish them a happy Thanksgiving, and then run from the room. Yeah. If the goal is an enjoyable holiday. And that’s the part many people don’t understand is what’s the point that you have to decide. If the point is to have a friendly conversation and you know one isn’t going to be had, then don’t have it. And you’re like, it’s that simple, Mike? It’s that simple. But it’s hard in the name of family, though, you know?

The takeaway is make the decision of what you want to do and have that mindset going in. If you’re determined to have a wonderful holiday, then don’t settle for anything less.

You know, at least in my neck of the woods, it’s been pretty mild. We’ve had a lot of 70s, which is pretty unusual since, you know, we live right by the Rocky Mountains. But we are winterizing our home, and it’s quite a few steps. I’m so thankful my husband is so handy. And so today we are going to talk winterproofing your home before the temperatures really start dropping and some simple steps that can prevent costly emergencies. I’d like to know about that. I hope you want to know as well, because here we go. Joining us is Meredith Sprecher, Director of Acquisition with American Home Shields. Yes, we have something old that I guess you can’t have in dry climates. We have a swamp cooler. First, when we bought the home, I was like, oh, great. I have to tell you, we love it. And it’s very efficient, at least for our house. But if you don’t cover up those vents, all that cold air just rushes in from the outside. And I can really feel it, you know, especially like in our bathrooms. I can feel that surge of cold air. So we have to do quite a bit, actually, to winterize our house. And we have two and a half acres. So there’s a lot of like… sprinkler system issues and freezing pipe kind of things that we have to take care of as well. So what are some ways that we can easily winter-proof our homes?
SPEAKER 03 :
Yeah, you’ve called out some great ones. There are simple things you can do around your home, like change out your air filters, seal your windows and doors, and insulate any exposed pipes. I know my husband and I just did that to our own home just last weekend. If you have a generator, it’s important to do things like check the oil, test the battery, and stock up on fuel before that winter storm hits. You can also take it a step further and call in a professional. If you haven’t already had a seasonal tune-up, now is a great time to do that. A seasonal heating tune-up is a great way to catch a problem before it turns into a costly and likely pretty cold situation. We don’t want anyone stuck without their heat this winter season and especially as we approach the holidays. You know, we’re talking about health care now, specifically site neutral payments that allow seniors to pay the same amount no matter where they are treated. Joining us is Jessica Burnell, director of federal affairs with Blood Cancer United. Welcome, Jessica. Thanks, Angie. Thanks so much for having me. Okay, so this is something very close to my heart. I have four senior parents in, you know, late 70s to mid 80s. And my mom just had what we thought was a stroke the other day, and she almost died. But we thought it was actually just low blood sugar. And literally, they had her back to normal, like almost comatose to back to normal once they gave her a bag of saline and, you know, they figured out what it was. Okay, so with that said… When we’ve taken her in in the past for different events, depending on where we are in the country, there’s such a wide variance in how much it costs, even like depending on the clinic. And there’s so much going on in health care now, which is very interesting to me, where it’s being run like a big business or like insurance is kind of handling, you know, what is paid and making arrangements. And it’s so confusing. And like the insurance is confusing. The payments are confusing. How much her insurance company pays is confusing. and then how much is covered by her Medicare, and I’m a college graduate. So this poor woman has no idea how to handle all this stuff. So thank you for helping us.
SPEAKER 06 :
Absolutely. You know, I handle this kind of thing for a living, and even I still find it confusing.
SPEAKER 07 :
Well, let’s just talk about site neutral. So first of all, explain what site neutral payments are and how it will help seniors specifically in terms of, you know, fair payments.
SPEAKER 06 :
Yeah, absolutely. So Site-neutral payment in the Medicare program is a really simple concept. It’s the idea that Medicare should reimburse for services at the same rate for the same service, no matter whether your patient visits a hospital-owned clinic or a clinic that is owned by an independent physician practice. Right now, the way that Medicare pays those two different types of clinics is quite different. And so, you know, you the patient can pay a lot more money for the same care just based on whose name is on the clinic door. And so site neutral payment reform would put an end to that and make that payment, you know, more equal across all of those clinics.
SPEAKER 07 :
Okay, so explain it to me. So Dr. Bob Smith used to own my doctor’s office, but now it’s owned by a big hospital. Does that make it more affordable for me in general? Or if the big business buys out the doctor’s office and it’s no longer owned by Dr. Bob Smith himself, is it less affordable?
SPEAKER 06 :
Yeah, so in that type of scenario, you’d be looking at care being less affordable. You’d see prices go up because Medicare will pay a hospital that owns that clinic more money for the same exact services than they did when, you know, Dr. Bob Smith owned that clinic independently.
SPEAKER 07 :
Wow. Okay. So unfair. All right. So let’s talk about how this, obviously it’ll help seniors save because if you’ve got cancer, for instance, or a chronic condition, you’d know that whether or not you went to a hospital owned clinic or an independently owned clinic, you’d be charged the same price, which to me, to be honest with you, it doesn’t even seem fair to me at all that Medicare would go like, look, Bob Smith, we would prefer to deal with a huge business and we’re not going to pay you as much. I mean, it seems so unfair, not only to the patient, but to the doctor. And so that’s what’s causing the payments to be higher for patients because of this change in who owns the clinics, et cetera.
SPEAKER 06 :
Yeah, exactly. Exactly. Yeah. It just depends on whose name is on the door. You know, everything else can be the same, same location, same doctor, same care that you’ve been getting. But just because the name has changed, all of a sudden you as a Medicare patient could see your costs go up.
SPEAKER 07 :
Well, and I think we’re all accustomed to, like, I’ve got three teenagers in sports, and we took our daughter in for what we thought was a stomach issue. Anyway, to make a long story short, after a couple of hours in the ER, they said, you know what, just give her ibuprofen and Tylenol, just rotate it. The bill was $13,000. So I looked at the doctor, and I’m like, that’s it? Like, just Tylenol or Advil, and we just rotate it? And then my son had, like, a serious ankle injury, took him to urgent care, and it was $50,000. And so like just the different – like right there, Advil, $13,000. Like are you kidding me? I mean I know they conducted tests, but that’s like just frightening to me that I could have just given her Advil at home. But a lot of people can’t afford a bill like that if they don’t have good insurance. So let’s talk about – hospitals obviously are opposed to site-neutral payments because that means they would make less money, right? So they’ve got a lot of pull. I know you worked a lot in Washington, D.C., so hospitals are fighting this.
SPEAKER 06 :
Yes, unfortunately, that’s the case. They understand that this would be less money going into their bottom line. But, you know, I think we, you know, our focus is always on what is actually best for patients and how much could patients actually save. You know, so we’ve crunched some numbers to try and get into a ballpark there. And, you know, for somebody with, let’s say, Multiple myeloma is a very common form of blood cancer. If you were to implement site-neutral payments, you’re looking at savings of about $1,200 a year for that patient, which, you know, is nothing to Medicare. It’s nothing to a hospital, but to a senior on Medicare, you know, who’s got a cancer diagnosis, you know, $1,200 can be extremely meaningful savings for them.
SPEAKER 07 :
Yeah, I mean, some of these, you know, seniors, at least where my mom lives, some of their checks are like 700 a month for Social Security. And if they worked in a factory, they may not have another form of income. That might be like what they attempt to live on, you know. So twelve hundred dollars is a couple of months of, you know, of living expenses for them. All right. So, Jessica, tell me what we can do, because I know bipartisan support can really help with this. How can how can we help with this issue?
SPEAKER 06 :
Yeah, so we’re really encouraged by the fact that this is a bipartisan issue. There’s support from both Republicans and Democrats in Congress, which, you know, is kind of increasingly unusual these days. You know, but there are lots of great bipartisan ideas in Congress that all the time, you know, that don’t actually make it to the president’s desk, that don’t become law. So, you know, it’s really important for folks to contact their representatives in the House and in the Senate and to talk about site-neutral payment reform and why their representatives in Congress should support it and prioritize it in whatever upcoming opportunities they have to vote on these issues. And so if folks would like to know more about site-neutral payments in Medicare and get some good information to convey to their members of Congress and also to get connected to their representatives directly, they can actually go to our website.
